ISACC (Integrated Services Assessment and Case Coordination)
"There are multiple "systems" that a client may find themselves in at one time - from legal to child services and we help to navigate and streamline what can be a complex and challenging process for clients."

quick facts
  • ISACC is a program that helps families who have been or are homeless by streamlining the process of moving out of shelter and into sustainable housing
  • ISACC is a highly collaborative and innovative partnership between several homeless-serving agencies created in order to move families permanently out of homelessness

ISACC works with families who have been homeless and who have complex needs. Within Calgary's 10 Year Plan to End Homelessness and the Housing First Philosophy, the ISACC Program plays an integral part in supporting families in assessing their barriers and needs, while also focusing on bridging the gaps between appropriate community services and navigating a variety of systems for each family. With proper supports in place, families are better able to stabilize and maintain their housing long-term.

Building on existing homeless-serving resources, a team of skilled and educated professionals provide case management for families experiencing homelessness through the coordination of community and mainstream service providers, to facilitate rapid re-housing, wrap-around support and the right matching of services to families' needs.

This modern approach to service coordination helps move families towards independence and out of homelessness permanently.
